Animal Contact Program
North Carolina State University’s Animal Contact Program is for the protection and education of individuals who have contact with animals. Environmental Health and Safety (EHS) oversees the Animal Contact Program in partnership with Occupational Medicine and the Institutional Animal Care and Use Committee (IACUC) for animal contact exposure. NCSU’s Occupational Medicine is the medical provider for the program and maintains all medical records (for employees only).

The Animal Contact medical monitoring must be renewed at least every three (3) years or when there are changes in:
- Work assignment (i.e., change in animal species, contact levels, etc.)
- Employee’s health status (development of a chronic illness or condition, immune suppression, pregnancy, development of animal or environmental allergies, etc.).
- Other hazards and work processes
What You Need to Get Started:
The animal contact application is hosted on OnBase, a secure, enterprise content management application. All new or first-time users, including supervisors and their employees, must obtain System Access Request (SAR) approval to access OnBase.
If you cannot access the Animal Contact Form with your NC State login credentials, you probably don’t have approved SAR access for this application. Please follow the steps below. Otherwise, please proceed to “Complete a new application” below.
For CVM Employees/Student Employees
Supervisors should send an email on behalf of themselves and their employee(s) to [email protected] requesting System Access (SAR) to complete the animal contact form on OnBase.
For CALS Employees/Student Employees
Follow the steps below if you are in CALS:
- Employees/student employees should first complete the Information Security & Privacy Acknowledgement (ISPA) form on the MyPack Portal. See instructions on How to Complete ISPA on MyPack
- The employee supervisor (not the employee) will then submit a formal ServiceNow request using this link. See instructions on How to Complete a ServiceNow Request
- The request will then be routed to the CALS business office.
- Supervisors will be informed by CALS Business Operations (CBO) when the request is approved.
All Other Departments
Please contact your department’s business officer, administrative staff or account manager to request SAR access for you (supervisor) and your employee. Note: The application is “OnBase”, while the role is “EHS – Employee Self Service”

Animal Contact Program Requirement
Program requirements are based upon those outlined in the Public Health Services Policy on the Humane Care and Use of Laboratory Animals, Guide for the Care and Use of Laboratory Animals (pdf), and recommendations of the Association for Assessment and Accreditation of Laboratory Animal Care (AAALAC).

Institutional Animal Care and Use Committee (IACUC)
Workers and supervisors with animal contact are subject ethical, practice, and care standards governed by federal and local laws, regulations and standards. Contact IACUC to ensure compliance with the university’s rigorous program of animal care and use.
